Warning:
You are advised that this is a “solicitation” for IRS Form W-9 information in
accordance with IRS Publication 1586. We are compiling records as required,
for the purpose of "establishing reasonable cause" and allocation of
penalties under IRS regulations.
We will soon be flagging your name and account at the IRS, by sending a
specially coded Form 1099 to the IRS. The 1099 will be filed electronically to
the IRS, with your name, address and other ID information. The 1099 will be
coded as missing your Social Security Number or EIN. We will be e-filing this
information to the IRS in approximately 2 weeks.
You will be responsible for the new IRS penalties, including the $500 penalty
and other sanctions including any applicable civil and criminal penalties for your willful failure to provide a Tax Id Number
and/or a correct name/TIN combination that match the IRS records. To avoid this
you can
provide the correct information on the enclosed IRS Form W-9*. For more
information, see the penalties section of Form W-9.
In addition to your penalties for failure to provide the correct W-9
information, you may be liable for negligence penalty or other sanction if this
income is taxable and the IRS determines that is has not been reported.
Please complete and return the enclosed W-9 in the pre-addressed return envelope
at your earliest convenience. Per Publication 1586, we are not required to
provide postage.
*The IRS penalties for missing Social Security Numbers also apply to TINs, EIN,
or any number which is obviously wrong or does not have 9 numeric
digits. The penalties also apply to 9 digit numbers that do not match the name.
If you have a business, corporation or trust, make sure the W-9 Name field (the
first line) is the same name used when you originally applied for your 9 digit
TIN or EIN. Do not use the dba or assumed name on the first W-9 name line.
